What is transgender top surgery?
For many transgender patients, cosmetic surgery is an important step in their transition that can result in decreased gender dysphoria, improved mental health and enhanced self-esteem. Alongside hormone therapy, top surgery is often one of the first steps for patients who are transitioning. At FORM Face + Body we offer gender affirming surgery for patients transitioning from assigned male at birth to female, as well as those transitioning from assigned female at birth to male.
For patients transitioning from male-to-female, transgender top surgery—or chest feminization—can be performed in order to create fuller, more feminine breasts. During the procedure, breast implants are surgically inserted beneath the breast tissue or the chest muscle, ultimately enhancing the size and shape of the breasts.

What are my transgender top surgery options?
Prior to any surgery at FORM Face + Body, you will see your surgeon for a consultation, which includes a comprehensive examination and explanation of your options, as well as a completely individualized treatment plan.
Before undergoing chest feminization via breast augmentation, your surgeon will review all options for creating your ideal breast appearance, including implant size, style, and shape (for some transgender women with a broader chest and shoulders, a larger implant may look more natural and proportional), implant material (silicone, gel, or saline), implant position (above or below the pectoralis muscle), and implant incision site.
For patients undergoing chest masculinization via elective cosmetic mastectomy, your surgeon will discuss and determine the best surgical technique to accomplish the desired breast tissue removal, depending on your unique anatomy and goals. The most common technique is a bilateral mastectomy with free nipple grafts, which allows for the removal of all breast tissue (including milk ducts), and excess skin, along with proper placement and downsizing of the nipples.
What are the risks of transgender top surgery?
All methods used for transgender top surgery are considered very safe, and most patients feel that the benefits outweigh the risks. However, our goal at FORM Face + Body is to make sure you understand what the surgery involves, including possible risks, complications and follow-up care.
Some of the issues you will discuss with your surgeon regarding chest feminization include:
- Capsular contracture (a tightening of the scar tissue which can make the breast feel hard and can distort the shape of the breast implant)
- Changes in nipple and breast sensation (usually temporary)
- Breast pain
- Rare implant leakage or rupture. If the implant is saline-filled, it deflates and the body harmlessly absorbs the saline solution. If the implant is filled with silicone gel, the gel may alter the shape of the breast, requiring implant replacement.
Some of the issues you will discuss with your surgeon regarding chest masculinization include:
- Diminished sensitivity or permanent loss of sensation in the nipples or areas of breast skin
- Irregularities or asymmetry
- The areolas (the pigmented area around the nipples) may appear uneven
- Seroma (fluid build up)
- Rarely, poor healing can cause scars to become thick and wide

Transgender Top Surgery
Our surgeons perform all transgender top surgeries in the hospital or at our private operating facility. Surgery usually takes one to two hours, is performed under general anesthesia and allows you to return home the same day.
Recovery will vary significantly between patients and depending on the type of procedure performed. Keep in mind that swelling and bruising are expected, so final results may not be evident right away. Your surgeon will provide you with customized guidelines to follow during your recovery, as well as oral pain medications and antibiotics based on your personal needs. In general, you can expect to return to normal activities by 2-4 weeks following your surgery. Your recovery will be gradual from day one after surgery and your surgeon will guide you through this process.
