FORM Campus was built as a single place for aesthetics, health and longevity. Twenty three thousand square feet of it at Yonge and Eglinton, designed around the idea that care shouldn't be scattered across six addresses.
Orientation Day is your first walk through it. Explore at your own pace, meet the physicians and teams behind each faculty, and stay for the ribbon cutting.
Two sessions, same programming. Choose the one that suits your day.
Doors open at the start of each session. Arrive when it suits you and stay as long as you like.
The campus is organised into five clinics. Walk through each one, see the spaces and the technology, and ask the teams anything.
Our surgical faculty. Meet the surgeons, see the operating and recovery spaces, and understand how surgical planning works at FORM.
Hair restoration, including the ARTAS robotic system and regenerative therapies. Meet the hair team and see the technology up close.
Assessment first care for pelvic floor health, postpartum restoration, menopause, sexual wellness and urinary concerns.
The FORM Men's Health Program. Hormone optimisation, body contouring, sexual health and recovery, in one dedicated space.
Longevity and wellness. Where assessment, prevention and performance come together.
Our physicians and clinical teams are on the floor throughout both sessions. Bring your questions.
Please gather a few minutes before. It's short, and it only happens once.
Every guest receives a FORM Campus Card on arrival. Your student ID, with a QR code that holds your exclusive offers from FORM. Keep it, scan it, use it. Orientation kit included: exclusive FORM branded cloth, varsity bag, notebook and pen.
Light refreshments are served throughout both sessions.
