Women’s health rarely sits in one lane. When clinically appropriate, your care plan may connect with FORM Life, Aesthetics, Hair, MSK, metabolic, hormone, and longevity pathways, so the right concerns are supported by the right team.

Pelvic Floor and Bladder Health: Support for leaking, urgency, frequency, pelvic heaviness, prolapse symptoms, and changes in bladder control.

Postpartum Restoration: Care for women recovering after pregnancy and birth, whether months or years later. This pathway may address pelvic floor symptoms, bladder leakage, intimacy concerns, scar or tissue changes, core recovery, and confidence returning to movement and daily life.

The Women’s Health Assessment is the entry point into FORM Women’s Health by Élevé, and the step that allows your care to begin with context, not assumptions.

Many women come to us with symptoms that overlap: bladder leakage, urgency, pelvic discomfort, painful intimacy, postpartum changes, recurrent urinary symptoms, menopause concerns, core weakness, fatigue, or changes in how they feel in their body. These concerns are often treated separately, even when they may be connected.

The assessment is designed to bring those pieces together. Your clinician will review your symptoms, health history, hormonal stage, pelvic and bladder health, sexual wellness, postpartum history, prior testing, lifestyle factors, and goals to better understand the full picture of your health. Where clinically warranted, this may also include a physical exam.

Every Women’s Health Assessment also includes baseline functional testing across body composition, lung function, and cardiovascular health.

From there, we help guide your next step, whether that means further diagnostics, pelvic health treatment, care management, regenerative options, hormone-related care, support within FORM, or a personalized Élevé pathway.

This is not a rushed consultation or a one-size-fits-all recommendation. It is a structured, physician-led starting point for women who want answers, options, and a care plan that reflects the full picture of their health. The goal is to connect what has often been treated in silos and create a plan that is specific to you, not generic.

Pelvic Floor & Bladder Health is for women experiencing symptoms such as leakage, urgency, frequency, pelvic pressure, pelvic discomfort, or difficulty trusting their bladder in daily life.

These symptoms are common, but they should not be dismissed as normal.

Postpartum Recovery is for women navigating changes after pregnancy or birth, whether recent or years later. This may include bladder leakage, pelvic heaviness, core weakness, abdominal separation, scar discomfort, pain with intimacy, or difficulty returning to movement.

We help assess what has changed, what still needs support, and how to rebuild function, strength, comfort, and confidence in a way that is specific to your body.

Menopause & Hormonal Health is for women navigating perimenopause, menopause, post-menopause or GSM symptoms, including changes in sleep, libido, bladder function, vaginal comfort, body composition, strength, and overall well-being.

You do not need to wait until symptoms feel severe to seek care. We look at hormonal stage, medical history, symptoms, risk factors, goals, and treatment preferences to help guide options that may include lifestyle support, diagnostics, hormone-related care, pelvic and bladder care, regenerative options, or broader FORM support.

Sexual Wellness is for women experiencing concerns such as pain with intimacy, vaginal dryness, reduced sensation, changes in desire or arousal, pelvic floor tension, postpartum changes, or menopause-related discomfort.

Sexual wellness is part of health, function, confidence, and quality of life. Sex shouldn't hurt. We provide a private, respectful, clinically guided space to understand what may be contributing to your symptoms and what options may help you feel more comfortable, connected, and in control of your body.

Hair thinning and hair loss can happen during major hormonal and life transitions, including postpartum recovery, perimenopause, menopause, stress, nutritional changes, and shifts in overall health.

Your assessment can help identify when hair changes may be part of a broader health picture. From there, your care plan may include women’s health support through Élevé, along with connection to FORM Hair and FORM Life where additional assessment, diagnostics, or treatment options may be appropriate.

Long-Term Health Optimization is for women who want to better understand their health beyond a single symptom. This may include changes in energy, strength, body composition, sleep, recovery, cardiovascular health, metabolic health, hormonal stage, or how they feel in their body over time.

Your Women’s Health Assessment, health history, goals, and baseline functional testing are used to support a more complete view of your health. Where appropriate, this may also include genetic mapping, advanced diagnostics, and connection into FORM’s broader campus resources.

From there, your clinician can help identify priorities, recommend next steps, and connect you with the right Élevé pathway and other FORM supports for proactive, personalized care built around your body, your goals, and your long-term health.

Meet Dr. Alaina Garbens

Founder & Medical Director, FORM Women's Health by Élevé

Dr. Alaina Garbens is a Urologist at Mackenzie Health and the founder of Élevé, a comprehensive health clinic dedicated to advancing whole-person care through regenerative medicine, functional wellness, and minimally invasive urologic treatment.

Élevé at FORM represents Dr. Garbens’ second location and expands Élevé’s women’s health offering into Midtown Toronto. This location is focused on Women’s Health across key stages of life, including pelvic floor and bladder health, recurrent urinary symptoms, postpartum recovery, genitourinary syndrome of menopause, sexual wellness, core and abdominal recovery, and long-term health optimization.

Dr. Garbens completed her medical degree, urology residency, and PhD in surgical education at the University of Toronto, followed by a two-year fellowship in minimally invasive robotic surgery and complex kidney stone surgery at UT Southwestern in Dallas, Texas. Her clinical approach blends academic excellence, surgical expertise, and a deep commitment to personalized, evidence-informed care.

FORM Women’s Health by Élevé, Dr. Garbens brings together urologic expertise, advanced diagnostics, regenerative therapies, and functional medicine principles to support women with concerns that are often treated separately or dismissed altogether.
